Adult ADHD Referral Process
Right To Choose Pathway
Due to extremely high demand, the NHS pathway for Adult ADHD assessments in Buckinghamshire is currently closed.
However, you still have the option to access care through the Right to Choose pathway. This allows you to be referred to an approved private provider, with the assessment and treatment funded by the NHS.
Please note that all Right to Choose providers are classed as private providers.
If you choose to pursue this option, it’s important to carefully select a provider. You can find a list of approved Right to Choose providers [HERE].
When selecting a provider, please ensure that:
- You meet their referral criteria.
- They are able to prescribe medication, if this is something you may require.
- You are aware of their current waiting times.
We understand the urgency many people feel about being assessed, but we strongly advise that you do not select a provider based solely on shorter waiting times. Please make sure the provider is suitable for your individual needs.
ADHD Referral – Required Documentation
To proceed with your ADHD referral, please ensure you complete all of the following steps and return the required documents to us:
- ADHD Screening Questionnaire – Please complete this in full.
- Personal Statement – Provide a detailed, handwritten or typed account describing:
- Your symptoms and concerns
- The impact on your daily life, including at home, at work, and in relationships
- Any strategies or interventions you’ve already tried
- What you hope to gain from a diagnosis
- Chosen Right to Choose Provider – Clearly state which provider you wish to be referred to.
- Referral Proforma – Download and complete the referral form from your chosen provider’s website.
- Please note: your GP cannot complete this form for you.
We are only able to begin processing your referral once all of the above documents have been submitted in full.
What Happens Next
Once we have received all required documentation, your information will be reviewed by a GP.
- If you meet the referral criteria, our administrative team will submit your referral to your chosen Right to Choose provider, along with all necessary documents. The provider will then contact you directly regarding the next steps.
- If you do not meet the criteria, we will inform you and offer guidance towards appropriate general support services.
Please note:
Once your referral has been submitted, the GP surgery will no longer be involved in the process. Any further questions or updates should be directed to your chosen provider.
