This information can be returned to us by either popping in with it or emailing it back to us at:

We will then be able to activate your account and provide you with your login details.

A separate form will need to be completed for any of your children: Consent to Proxy Access to GP Online Services (PDF).

You will then be able to go to the Patient Access website and fully register your account: Patient Access website.
