Respiratory Nurses

Women’s Health Nurses

The team is trained in managing both routine and more complex women’s health concerns, working closely with GPs and other healthcare professionals to ensure holistic and patient-centred care. Complex Care Team

The Complex Care Team (CCT) at Westongrove is a multidisciplinary group—including nurses, healthcare assistants, paramedics, social prescribers, and care coordinators—working alongside GPs and local services to support patients. Taking a holistic approach, the team addresses health monitoring and management, financial guidance, social wellbeing, home safety, and emotional support, ensuring patients can live safely and independently at home. They also lead advanced care planning discussions, covering end-of-life wishes, hospital admissions, resuscitation preferences, and legal documentation, all delivered with compassion to provide the right care at the right time.

Social Prescribing

Our Social Prescriber at Westongrove plays a key role in supporting patients with non-medical needs that impact their health and wellbeing. By taking time to listen and understand what matters most to each individual, the social prescriber helps connect patients to local services, community groups, and activities that can improve their quality of life. This may include support with loneliness, housing, employment, financial concerns, carer support, or emotional wellbeing. Working closely with GPs and the wider healthcare team, the social prescriber empowers patients to take control of their health by addressing the wider social and practical factors that influence it. This holistic approach supports better outcomes and helps people feel more confident and connected in their everyday lives.

Paramedics

Our Paramedics at Westongrove are highly skilled clinicians who support the assessment and management of patients with urgent or complex health needs. Working both in patients’ homes and within the surgery, they carry out thorough clinical assessments, provide treatment for acute illnesses and minor injuries, and help manage long-term conditions. Paramedics also play a key role in reducing unnecessary hospital admissions by delivering timely care and making appropriate onward referrals when needed. As part of our multidisciplinary team, they work closely with GPs, nurses, and the Complex Care Team to ensure patients receive safe, responsive, and person-centred care.

Pharmacy Team

Our Pharmacy Team at Westongrove are an integral part of our healthcare team, providing expert advice on medicines and supporting safe, effective prescribing. They conduct medication reviews, help manage long-term conditions such as high blood pressure and ensure patients are getting the most benefit from their treatments. The pharmacy team also support repeat prescription requests, answer medication-related queries, and help with the management of side effects and interactions. By working closely with GPs, nurses, and other healthcare professionals, our pharmacists contribute to high-quality, patient-centred care and play a vital role in optimising treatment and improving health outcomes.
