Register with the Surgery

If you have recently come to live in Aylesbury, Wendover or the surrounding area and are eligible to register with the surgery you can complete the attached forms and bring them in to the surgery to register. Alternatively, you can collect the registration forms from any of our surgeries.

All newly registered patients are invited to make an initial appointment to discuss their health needs. Please speak to a member of the reception team if you wish to do so.

Practice Boundary

You will also need to reside within our practice boundaries.

Each practice within Westongrove has its’ own catchment area.

Wendover Health Centre covers the villages of Wendover, Weston Turville, Halton and the Lee. If you live in Stoke Mandeville, Aston Clinton, Cholesbury, South heath and Little Kimble, please check with us first as we only cover certain parts of these villages.

Bedgrove Surgery covers the south east of Aylesbury including Bedgrove, Broughton Estate, Walton Pond , Stoke Grange & Elm Farm. If you reside within the area that is south of the canal, to the east of Victoria Street, Walton Road, Stoke Road & Mandeville Road, and to the west of Hampden Hall on the Wendover Road and the Holiday Inn on the Tring Road, you should be able to register with us. If you are unsure, please give us a call. Bedgrove Surgery cover certain parts of Stoke Mandeville, but please check with us.

Aston Clinton Surgery covers the villages of Aston Clinton, Tring and surrounding villages such as Weston Turville, Buckland, Pitstone, Cheddington, St Leonards, Marsworth, Long Marsden, Puttenham, Ivinghoe. If you live in Wiggington, Halton, Broughton and Bierton, please check with us first. We also cover parts of Aylesbury to the southeast from A41 Bedgrove Junction.

Who is Registering?

Please print off and complete all of the below forms and bring them in to the surgery together with your proofs of ID. In order to process your application efficiently, we advise you to obtain your NHS number which you can get from your previous GP.

Required ID

We require two forms of ID which you must present in person – one for proof of identity and one for proof of address. All forms of ID must be valid and proof of address must be no more than three months old.
